I am an engineering student developing a new software that will automatically check ultrasound order forms for errors. I am trying to get a sense of what sonographers need from this technology.

1) What are the most common errors that you see on order forms that force you to contact the doctor and get a new order form?

2) How often do these errors occur? How big of a problem do they present to you?
